Our Community Services provides information, education and specialised support to people with dementia | mate wareware, whānau (family) and support workers in the Hawke’s Bay Region.

• Meet with you and/or your whānau (family) either at home or in our rooms
• Discuss your situation and help you make a plan to manage upcoming changes
• Provide information and education about Dementia
• Liaise, support and help you navigate other services agencies, and professionals integral to your care
• Review your individual support plan and make changes when needed to ensure you get the night support to achieve the best possible outcome
